— On February 14, 2006, Employee #1 was working from scaffolding that was approximately 30-feet tall. The scaffolding was directly above a drilling rig cellar that was about 10-feet deep. While Employee #1, thought to be collecting tools, was on the scaffold, he fell into the cellar, killing him. Employee #1’s hard hat was found on the scaffolding directly above the cellar, with Employee #1 below. The accident investigation revealed that Employee #1 was not wearing fall protection, and the injuries found on his body were consistent with a fall from a scaffold. There were no witnesses.

We still see metal sheets (mostly imported from Europe or the USA north-east) which consist of flat sheets, with a single raised rib every 25 to 40 centimetres, screwed down in the flat section of the sheets, with relatively small screws, which come with a tiny rubber ring, and a small washer. Under normal conditions, these sheets are often a problem because rainwater will penetrate trough the screw opening when after a few years, the rubber ring has deteriorated, and in hurricane conditions, the screw and small washer are simply pulled through the sheet. The building has vaulted ceilings as well as a loft for additional space. The interior also houses three animal pens to keep therapy animals. Lakemary has a completely lined interior with partial loft. The liner is 2 X 6 T&G that was produced by Midwest Columns, LLC. All exterior Dutch doors and cupolas were ordered through CannonBall:HNP. The stone veneer wainscoting was ordered and installed by Canyon Stone Inc., Olathe, Kansas. The customer was interested in a European-style stall. The stall fronts and dividers were ordered through Saratoga Stalls, LLC,Grand Rapids, Iowa.
